Case of the Month #34: Shoulder Pain by Dr Sunil Dasari

Published: 14/12/2023

Assessment

  • Q1: What is the diagnosis?

B) Impingement syndrome

  • Q2: Range of movement which is painful in impingement of shoulder?

D) 60-120 degrees. A classic presentation is of a painful arc on movement when the arm is lifted out to the side. The movement is painless up to 60 degrees and after 120 degrees.

  • Q3: How do you diagnose Impingement syndrome?

C) History and physical examination

  • Q4: Only Localised injection resulted in a maximised treatment effect.

False. Physiotherapy plus localised injection resulted in a maximised treatment effect compared to just localised injection.

  • Q5: If the conservative treatment fails, surgical decompression gives the best results.

False. A systematic review of randomised controlled trials comparing surgical intervention versus
conservative therapy yielded moderate evidence that surgical intervention was not more
effective for reducing pain than impingement-directed physical therapy.

What is the differential diagnosis?
Impingement syndrome, adhesive capsulitis, rotator cuff tear, acromioclavicular joint arthritis, glenohumeral arthritis, acromioclavicular joint sprain, biceps tendonitis, biceps tendon rupture, cervical radiculopathy, thoracic outlet syndrome. The pain here is due to impingement syndrome.
