Management Plan

She had antibiotics for community acquired pneumonia. For analgesia, she received a fascia iliaca block (FIB) on admission followed by regular intravenous (IV) paracetamol and oral morphine (regular and as required doses of 2.5mg). She remained delirious but mostly settled except for when she had to be moved. It was decided to supplement her analgesia with a local anaesthetic infusion via femoral nerve catheter which was effective.

Her pneumonia and cognitive function gradually improved and, a week later, she had a Dynamic Hip screw (DHS) under spinal anaesthesia. She continued to improve and was subsequently discharged to a nursing home.
